Include: patient demographics, diagnosis, prescription for pneumatic compression device (E0651 or E0652), and insurance information.

We Verify & Authorize

We verify the patient's insurance benefits and manage prior authorization. If additional clinical documentation is needed, we'll contact your office directly.

Delivery & Training

We deliver the device, fit the garments, and train the patient on proper use. Your office is notified when the patient is set up.

Required Documentation

Physician prescription / order for pneumatic compression device
ICD-10 diagnosis code(s) for lymphedema or qualifying condition
Face-to-face evaluation notes documenting medical necessity
History of conservative treatment (e.g., manual lymph drainage, compression garments, elevation)
Patient measurements for garment sizing (we can assist with this)
Patient insurance information and demographics
